Brain Health: What the research shows
Cocoa flavanols have attracted growing scientific interest because the same vascular effects observed in cardiovascular research may also influence the aging brain. Over the past two decades, researchers have investigated whether improvements in blood flow and vascular function translate into measurable changes in memory, attention, and executive function.
The results, while extremely promising—are not yet conclusive.

Executive Function
Gratton et al. (2020) showed dietary flavanols increase cerebral cortical oxygenation and improved performance on demanding cognitive-control tasks in healthy adults. The benefit tends to be biggest under cognitively demanding or fatiguing conditions — mental fatigue, sleep loss, or sustained effort — where flavanols help sustain performance rather than boost an already-rested brain.
Memory
Large long-term trials have not shown significant improvements. The COSMOS program (three-year RCTs, thousands of older adults) did not find an overall memory benefit from cocoa flavanol supplements. The one durable signal was in a subgroup: people who started with low diet quality / low habitual flavanol intake saw memory restored.
Cerebral Blood Flow
Among the most consistent findings in cocoa flavanol research is improved cerebral blood flow. Human imaging studies have shown measurable increases in blood flow and oxygen delivery to the brain, with effects observed both shortly after consumption and following regular daily intake.

How Cacao May Support Brain Function
The same vascular effects that make cocoa flavanols a focus of cardiovascular research have also attracted interest in brain health. By increasing nitric oxide availability, flavanols may improve blood vessel function and support healthy cerebral blood flow.
Researchers have been especially interested in the hippocampus—a region of the brain involved in learning and memory. Some imaging studies suggest flavanol-rich cocoa may increase blood flow and activity within the dentate gyrus, an area that naturally becomes less efficient with age.
These findings provide a biologically plausible explanation for why some clinical studies have reported improvements in certain aspects of cognitive performance.

What human studies have found
Early randomized clinical trials produced encouraging findings. Several studies reported improvements in memory, executive function, processing speed, or cerebral blood flow following supplementation with flavanol-rich cocoa.
One landmark imaging study published in Nature Neuroscience found improvements in both dentate gyrus function and memory-task performance among older adults receiving high-flavanol cocoa over several months.
However, larger and longer-duration trials have produced more mixed results. In the COSMOS-Mind study, cocoa flavanol supplementation did not significantly improve overall memory or global cognition across all participants. Together, these findings suggest that while cocoa flavanols may influence certain aspects of cognitive performance, the effects are not consistent across every population studied.

Why processing matters
Not every cocoa or cacao product contains the same amount of flavanols. Fermentation, roasting temperature, storage, and other manufacturing decisions can all affect the concentration of these naturally occurring compounds.
Alkalization—often called Dutch processing—can substantially reduce flavanol content. This is why research findings involving specially prepared, flavanol-rich cocoa cannot automatically be applied to every chocolate bar, cocoa powder, or cacao product on the market.

Current scientific consensus
Current evidence suggests cocoa flavanols may support healthy cerebral blood flow and certain aspects of cognitive performance. The strongest signals have been observed in smaller studies and among individuals with lower baseline flavanol intake.
At the same time, the largest randomized clinical trials have not demonstrated a consistent overall improvement in memory or global cognition. Researchers generally view the field as promising but still evolving, with additional studies needed to determine which populations are most likely to benefit.
