What the Research Says About Heart Health

Among the many areas of cacao research, cardiovascular health has received the greatest scientific attention. Human trials have examined how naturally occurring cacao flavanols may affect circulation, blood-vessel function, endothelial function (lining of blood vessels), and blood pressure.

How cacao may support circulation

The inner lining of the blood vessels is called the endothelium. It helps regulate circulation by producing nitric oxide, a signaling molecule that tells blood vessels when to relax and widen.

Cacao flavanols appear to stimulate nitric oxide production and help protect nitric oxide from breaking down too quickly. This may allow blood vessels to respond more effectively, supporting blood flow and normal vascular flexibility.

What human studies have found

Randomized human trials have repeatedly reported improvements in endothelial function after participants consumed cocoa or cacao products containing meaningful amounts of flavanols. Researchers commonly measure this through flow-mediated dilation, which evaluates how well an artery widens in response to increased blood flow.

Some trials and meta-analyses have also reported modest reductions in blood pressure, particularly among people who began with elevated readings. Results vary by population, dosage, study duration, and the flavanol content of the product being tested.

Why processing matters

Not every cocoa or cacao product contains the same amount of flavanols. Fermentation, roasting temperature, storage, and other manufacturing decisions can all affect the concentration of these naturally occurring compounds.

Alkalization—often called Dutch processing—can substantially reduce flavanol content. This is why research findings involving specially prepared, flavanol-rich cocoa cannot automatically be applied to every chocolate bar, cocoa powder, or cacao product on the market.

Current scientific consensus

The evidence is strongest for a short-term improvement in endothelial function following the consumption of adequately dosed cacao flavanols. Research also suggests the possibility of modest blood-pressure benefits, although the size and consistency of the effect differ between studies.

Cardiovascular research on cacao is promising, but it should not be interpreted as proof that cacao prevents heart disease. The most responsible conclusion is that flavanol-rich cacao may support normal vascular function as one part of an overall healthy diet and lifestyle.

THE COSMOS TRIAL

Reduction in Cardiovascular Death

In the COSMOS randomized clinical trial involving more than 21,000 older adults, participants receiving cocoa flavanols experienced a 27% reduction in cardiovascular death compared with placebo during a median follow-up of 3.6 years.

The study did not find a statistically significant reduction in its primary composite cardiovascular endpoint, but cardiovascular mortality was significantly lower in the cocoa flavanol group.

This finding represents a statistically significant secondary outcome from the COSMOS trial and should be interpreted within the context of the study as a whole. Additional research is ongoing.
